Vehicle examiner Wilf Johns said an immediate prohibition was issued to a vehicle presented for annual test in November for three brake defects He examined two vehicles in December, issuing two defect notices. Mileages were not recorded on the inspection sheets and no driver defect reports could be produced.
Seven immediate and two delayed prohibitions had been issued since a previous public inquiry in 1990. Maw was sent a warning letter about maintenance in January Maw said that now the operation had been scaled down, they did not require books as the defects were reported verbally. Statements like that would quickly lose Maw his licence, commented Waterworth. Maw assured the LA that he would introduce a system of written defect reports.
Cutting the authorisation to three vehicles, and directing that the licence expire at the end of the year instead of in October 1996, Waterworth said that there had been a significant maintenance failure.
