In the annual reports of the Licensing Authorities, 869 cases are quoted involving maintenance, and having perused all the cases reported in CM over the period, also reports on accidents, it is evident that the majority were due to lack of inspection and also lack of inspection records.
In your own excellent White Paper Casebook article (CM, April 22) I quote as folows:
